New Patient Registration 

The Practice welcomes new patients residing within the Practice defined boundary. Patients will need to complete registration forms before they are able to request an appointment with the GP or Practice Nurse

In order to register with the practice you can:

• complete the pre-registration form which you will find on the pre-registration tab
• download the registration form which is also under the pre-registration tab
• collect registration forms from the practice reception 

You will need your NHS Number to be able to register, this you can find on a prescription reorder form from your previous surgery, hospital letter or you will need to contact your previous surgery.

All new patients will be asked to complete a health questionnaire (see the next tab on pre-registration) and attend an appointment for a health check with the Practice Nurse to complete the registration. This will allow the practice to collect information on your past medical health prior to receiving your full medical records.

Repeat Prescriptions - patients on repeat medication will be required to see the practice pharmacist before they order their first prescription in order for medication to be checked and updated on to their medical records.
